Age:70 years.

Weight: 70 Kg.

Reason of visit: Fatigue,nausea, Wt loss,lower extremeties pain .

Dignosis: D.M, HT.

Past drug history:dionil ,enalapril.

Current drug:
1-Insulin 35 IU\day: 20 IU at morning and 15 IU at evening.

2-Enalapril 2tab \day.


Drug -therapy problem: related with effectivness: use of Insulin instead of dionil because insulin is more effective drug.
